Forensic Case Management

Our Forensic Case Management services are available to people aged 16 or older with a serious mental illness residing in Elgin, Middlesex, Grey-Bruce, Essex, Kent, Lambton, Oxford, Huron, or Perth Counties, or in Kitchener-Waterloo Region. The service is offered in collaboration with the Southwest Centre for Forensic Mental Health Care. Forensic services include assessment, treatment, and rehabilitation of individuals with serious mental illness who have been in significant contact with the law.

Our Forensic (Transitional) Case Manager assists with the successful transition of forensic unit inpatients to community placement through counselling, follow-up, crisis intervention, and community service linking.

Hours are Monday through Friday, 8:30AM to 4:30PM. Additional support is available through our Crisis & Intake team.
